1. The Clinical documentation Improvement specialist facilitates modifications to clinical documentation through concurrent (pre-bill) interaction with physicians and other clinical staff.
2. Evaluates the clinician documentation, lab results, diagnostic information, and treatment plans and communicate with the physician either through discussion or in writing regarding missing, unclear, or conflicting health record documentation, and clarify the information as warranted
3. Educate physicians about identification of disease processes that reflect SOI, complexity, and acuity in order to facilitate accurate application of code sets
4. Gather and analyze information pertinent to documentation findings and outcomes, and use this information to develop action plans for process improvemen
