Job Description :
â€¢To assess and manage diabetic patients with special ability to identify complications.
â€¢Practices in accordance with a patient self-management model to assist people living with diabetes to achieve their optimum level of health and quality of life.
â€¢Partners with patients and their families to enable them to design and manage their own individualized diabetes protocols / plans (e.
g. establishes goals with patients)
â€¢Collaborates directly with clinicians to form comprehensive care plans with the patient.
â€¢Provides blood glucose monitoring training and pattern management guidance to patients. Suggests medication adjustments to achieve optimal glycemic control and recognizes inappropriate medication orders.
â€¢Provides insulin teaching and titration as prescribed by PCP.
â€¢To take a leading role in patients and family education.
â€¢To liaise with other healthcare personnel to ensure continuous efficient patient education and life style management.
â€¢To maintain high standards of documentation of care and activities.
â€¢To participate in the setting, monitoring and improvement of standards and the quality of patient care.
â€¢To undertake and participate in specialist clinical supervision.
â€¢To maintain a safe working environment.
â€¢To provide information of present needs and future trends for service delivery.
â€¢To demonstrate personal responsibility for compliance with infection control policies and procedures.
â€¢To maintain record of own workload and performance monitoring information and review this on a regular basis with the Unit Manager.
â€¢To actively participate in relevant national meetings / events to stay abreast of new developments and advances within the speciality area and feedback this information to the team to ensure continued learning and improvement to the service.
â€¢To contribute to policy and protocol development for speciality area.
â€¢To exercise a personal duty of care in relation to equipment, resource and stock control utilizing equipment and supplies appropriately.
â€¢To use advanced communication, counselling and interpersonal skills when delivering sensitive information, breaking bad news to patients and / or carers, or dealing with aggressive behaviour and contentious situations.
